The downside of pressure filling has always been, that there is no real good way to determine just when the cement gets to the apex.
Thus since we haven't been able to figure this out either, we have stuck with passive filling.
After the syringe has been fine tuned to make sure that the smallest clockwise turn of the plunger starts the flow and the smallest turn counter-clockwise stops the flow we introduce the needle to within one millimeter of the apex.
